Subject: Referral for Transplant cases standard instructions in HMIS based digital referral protocol

An instance regarding referral for a transplant case given by a Railway Hospital was found to have been issued mentioning "Referred procedure/ treatment:. ..workup..." and "Type of Service: treatment /procedure (outdoor) (approved session: 1 no.)". The ambiguity /incongruity caused considerable confusion and inconvenience to the patient and the empanelled hospital.

2. Transplant cases, being highly complex, constitute a unique category of referrals. Unlike other referrals, all transplant cases essentially undergo four broad steps/processes viz. Workup tests Medico-legal/NOTTO compliance Transplant procedure Follow-up. Workup tests are a complex battery of tests on the patient/donor spread over several sessions over a period of time. The transplant procedure is dependent on and decided based on the workup tests. Hence, the locus of workup or transplant procedure cannot be at different empanelled HCOs and must necessarily be covered under a single comprehensive referral.

3. In order to preclude repeat of such instances or error in issuing such referrals, the following has been incorporated as standard/dropdown auto-select menu in the HMIS referral /doctors' desk (from where digital. referral is generated in HMIS) and should be ensured/ rechecked in all transplant referral cases:

L Specialty: (to be picked from drop down menu in HMIS e.g., kidney/liver/lungs etc.)

IL Referred procedure/treatment: "Transplant with workup" (drop down menu in HMIS)

iii. Referring doctor's remarks: (clinical remarks to be recorded).

iv. Type of service:(auto select in HMIS, if referred procedure/treatment is as per ii. above) -"OPD/IPD for comprehensive treatment including follow up". [note: Kidney transplant cases would be inclusive of the dialysis required during the entire treatment period.)

Footnote in all transplant cases: Applicable CGHS package and due compliance with medico-legal /NOTTO requirements to be ensured by the empanelled HCO. Dialysis for kidney transplant cases may be billed separately as per CGHS rates (if not a part of the transplant package rate) or the patient may claim reimbursement for such dialysis.

This issues with the approval of DG(RHS).

Sub.: Promotions to Group ‘B’ posts on Indian Railways through Centralized Computer Based Objective Type Examination — 30% LDCE for the vacancy cycle 01.01.2025 – 31.12.2026.

Ref.: Board’s letters of even number dated 18.02.2026, 19.02.2026 and 26.02.2026.

The Centralized Computer Based Test (CBT) for 30% LDCE for promotion to Group ‘‘B’ posts in departments having Organized Services for the vacancy cycle 01.01.2025 to 31.12.2026 shall be conducted on 24.05.2026 and 31.05.2026, with 50% of the candidates from each Department appearing on each date. As already advised, it may be ensured that the bifurcation of candidates to appear in the Examination on the said two dates is intimated to RRB/Ajmer well in time. In view of the necessity that has arisen due to operational requirements to conduct the exam on two dates, it has been decided to follow normalization process to ensure parity between both sets of candidates. This may be duly indicated while notifying the fresh exam dates.

Night Duty Allowance Calculation

Night Duty allowance will be calculated as per following method, the following 4 values required to calculate NDA

1. Weightage Hours


Weightage of 10 minutes for every hour of duty performed between 22:00 Hrs and 06:00 Hrs will be taken for calculation of payment of NDA, which means In the Night Duty every one hour is considered as 10 minutes to calculate NDA

Example, an employee worked 8 hours in Night Duty, then weightage hours will be 80 minutes (10 Mins x 8 hours)

Attention of the Railways is drawn to Railway Ministry’s Decision No. 11 below rule 18 of Railway Services (Conduct) Rules, 1966 read with clarification issued vide Board’s letters No. E(D&A)2007 GS1- 1 dated 28.3.2008 and 17.6.2009, providing that Supervisory Staff of Group ‘C’ on Railways working in scales of pay the maximum of which is Grade pay of Rs. 4600 and above are required to submit an annual return regarding the immovable property inherited by him, or owned or acquired by him, or held by him on lease or mortgage, either in his own name or in the name of any member of his family or in the name of any other person. The return should be submitted within three months of his first appointment in the grade and thereafter in the month of January every year.
